Patient Support Services

Patient support services are complementary to the primary care program, and focus on those segments of our patient population requiring greater attention - the elderly, pregnant women and new families, the homebound, and people with chronic illnesses such as AIDS or diabetes.

The Patient Support Services team offers the following services:

• Diabetes education and disease management services to patients referred by one of our providers
• Home visits are made by referral from our medical staff
• Nutrition counseling is available to all WIC clients and to medical clients referred by one of our providers
