How do I make an appointment?

Call the clinic's appointment line and give the scheduler a brief explanation of the reason for your visit and the name of your primary care provider.

What should I bring to my first appointment?

You should bring your insurance cards or medical coupons, immunization records, previous medical records (if you have them).  You will also be asked for your current address and telephone number.

Can I be seen at either clinic?

We ask that you choose one clinic and receive all of your medical care there because that is where your medical records are maintained.

Occasionally your provider may refer you to the other clinic for special services.
